Avenue 360 Health &amp; Wellness will continue to provide OBGYN services, and I hope you will continue to utilize their services. Patients may continue to remain at A venue 360 for their OBGYN Care services, or they may choose to establish care with another physician\/provider. For patients: medical records are confidential; however, a copy can either be transferred to another physician\/provider, with a signed written authorization form from you, or released to you with proper identification. If you choose to see a different physician\/clinician, please request an authorization form, and return it to our clinic as soon as possible, so we may transfer your records to your new physician\/provider. Until then, your records will remain on file at<br \/>\nAvenue 360 Health &amp; Wellness.<\/p>\n<p>Thank you for entrusting your medical care to me and Avenue 360 Health &amp; Wellness. We both appreciate the opportunity to assist in your care.
