This letter is to inform you, that my last day with Avenue360 Health & Wellness will be Monday, December 2, 2024. Avenue 360 Health & Wellness will continue to provide you with pediatric care, and I hope you will continue to utilize their services.
\nPatients may continue to remain at Avenue360 for their Pediatric Care Services, or they may choose to establish care with another physician\/provider. A team member will be reaching out to you shortly to schedule an appointment with one of my colleagues here at your child\u2019s clinical home to help ease this transition.<\/p>\n
For Patients wishing to seek care elsewhere; including either with one of our other clinical locations: Medical records are confidential; however, a copy can either be transferred to another physician\/provider, with a signed written authorization form from you, or released to you with proper identification. If you choose to see a different physician\/provider outside of Avenue 360 Health and Wellness, please sign an authorization form and return it to our clinic as soon as possible so that we may transfer your records to your new clinical home. Until then, your records will remain on file at Avenue 360 Health & Wellness.
